A 38 year old man with breathlessness and vomitings

 A 38 year old gentleman, daily wage worker , resident of reddibavi came to hospital on 6/9/22 with 

CHIEF COMPLAINTS OF 

Vomitings and pain in abdomen since 5days and breathlessness since 4 days.

HISTORY OF PRESENT ILLNESS 

The patient was apparently assymptomatic 5 days ago . Then he had pain in the abdomen which was twisting in nature and vomitings which were continuous , non bilious , projectile  and watery in consistency. Then he had breathlessness since 4 days. 

PAST HISTORY 

He has DM from one and half year

No HTN 

No TB , CAD , Epilepsy , asthma 

No past surgeries  


PERSONAL HISTORY

Married

Mixed diet 

Inadequate sleep

Normal appetite

Normal bowel movement and urination 

Occasionally alcoholic 

Smoker 

No history of any drug allergy

No known allergies 


FAMILY HISTORY 

Father is diabetic 

Father is hypertensive 


GENERAL EXAMINATION 

He is conscious , coherent 

He is moderately built and moderately nourished

No pallor  

No icterus 

No cyanosis , clubbing of fingers , no lymphadenopathy

VITALS 

Temperature -98.7F

Pulse rate- 100bpm

Respiratory rate-28cpm

BP- 90/60 

SpO2-95% 

Grbs -High

SYSTEMIC EXAMINATION

CVS 

S1 S2+

CNS 

Normal 

INVESTIGATIONS

TREATMENT

1.IV 5%dextrose @100ml/hr

2.INJ  HAI 1ml 

3.INJ PAN 40mg /IV/OD/BBF

33 year old female,  resident of west bengal came to hospital with  CHIEF COMPLAINTS Pain radiating from groin to right flank region gradually increasing since 3 years. C/O nausea and vomiting since 3months No complaints of fever  HISTORY OF PRESENT ILLNESS  Patient is apparently asymptomatic 3 years ago.
